Prior to the start of services we kindly ask that you complete the New Client Packet. Completing these forms prior to your first visit will ensure a quick start to services. These forms provide your clinician with vital information regarding your current circumstances and concerns. During your initial evaluation (typically lasting 45-60 minutes), a detailed psychological and medical history will be obtained. You and your clinician will determine target goals for therapy and formulate a treatment plan to best address your needs. During this time we will also answer any questions you may have about the therapeutic process. Should it be deemed appropriate, we will partner with your general practitioner or other medical professionals to assure continuity of care. Follow up sessions will be scheduled according to the treatment plan that you and your psychologist have established.
We at Esteemed understand that each child is unique and deserves personalized attention. For any inquiries you may have regarding testing or therapy for your child, please contact us. We will be happy to provide you with a detailed description of our services. We aim to assist you with making an informed decision on when and how to begin the therapeutic process.
Although we work closely with a variety of insurance carriers, we cannot guarantee all services will be covered by your provider. Many insurance companies provide out-of-network benefits, which may cover a percentage of your fees for psychotherapy. In these circumstances, we are happy to provide you with a receipt of payment that you may submit to your insurance company for reimbursement. Prior to the initial visit, please contact your insurance carrier to determine mental health benefits, deductibles and co-pays.
